Committee/Division Meeting Requests


Starting with the 2020 Annual Meeting all division and committee meeting requests for space during National Meetings must be submitted via the attached form. Please complete this form with the details for your meeting, and hit submit or you can save and attach and send it to  After submitting your form, you will receive a confirmation of receipt.  If you don’t receive a response within 24 hours, please resend.

List any questions or comments in the notes section. You will be contacted if we have questions, or to answer your questions once we start assigning meeting rooms.

If we don’t receive a form or an email, space will not be assigned.
